IU Health Medical Group South Region is hiring! We are seeking a Nurse Practitioner to join our Hospice/Palliative Care team at the Hospice House in Bloomington, Indiana.
IU Health Palliative Care is focused on improving quality of life while dealing with a serious illness and providing personalized care that is focused on the patient, not just their disease, supporting care goals through pain and symptom management, spiritual and psychosocial counseling and assistance in advance care planning. We aim to support the patient and their loved ones.
Position Details:
Monday through Friday, 8:00am to 5:00pm - (rounding within 24 hours of admission)
Provide clinical consults with patient/family members and occasional phone consults (as needed)
Hospice House average 10 patients per week
Home visits average 5-7 patients per week (counties include Monroe, Brown, Owen, Lawrence, Orange and Greene)
Key responsibilities include: end of life care, medication management, H&P/clinical notes and symptom control
Clinical team includes one collaborating Physician; Dr. Brad Bomba and 6 Advanced Practice Providers
Documentation and charting will be completed in Homecare Homebase (HCBH). On-site training will be provided.
Qualifications:
Must hold FNP certification
APP Experience in Palliative or Hospice Care is required
Must have valid driver's licensure and reliable transportation as driving is required
