Nemours is seeking a Lymphatics APP to join our Wilmington Team!
The Lymphatics Program APRN/PA will function in both an independent and expanded collaborative role by planning and providing comprehensive management to patients with lymphatic disorders. The patients will be seen in the inpatient and outpatient setting. The provider must work collaboratively with an interdisciplinary team and demonstrate excellent written and verbal communication skills in all phases of practice. The ideal candidate will be an educator and develop longer-term clinical relationships with patients with lymphatic disorders and their families.
Primary Functions:
Assessment, diagnosis, management, and follow up visits for lymphatic disorder patients with acute and chronic needs.
Provide concise documentation of patient encounters including diagnoses, management strategies and follow up plan.
Identifies patient and family needs for additional resources and initiates appropriate referrals.
Provides patient and family teaching based on identified learning needs.
Evaluates compliance with the prescribed treatment regimen through phone calls, emails, app messaging, patient/family assessments, and outpatient visits as needed.
Manages all lymphatic cases regarding assessment and coordination - determines appropriate records, records review, requests new records and develops case presentations for case reviews.
Requirements:
Graduate of an accredited Pediatric Nurse Practitioner Program(or) Graduate of an accredited Physician Assistant Program
Prior clinical experience with pediatric cardiac patients preferred.
APRN:
Current unrestricted Advanced Registered Nurse Practitioner License to practice in the state(s) in which privileges are sought
Pediatric Nurse Practitioner Certification via National Certification Center
AHA BLS and PALS Certification upon hire.
DE RN (or compact state), DE APRN, DE CSR, DE DEA
PA-C:
Current unrestricted Physician Assistant License to practice in the state(s) in which privileges are sought
National Certification from the National Commission on Certification of Physician Assistants
