To provide patient assessment, patient education and rehabilitative services to patients in the Cardiac Rehabilitative program.

Responsibilities:

• Performs a thorough and ongoing nursing assessment. Identifies and prioritizes current and potential problems. Re-evaluates problems as needed. Develops appropriate intervention and desired outcomes for all identified problems.
• Collaborates with appropriate healthcare team members, patient and family/significant others to meet patients present and discharge needs. Implements plan of care at admission to program. Evaluates, revises, and documents the patient plan of care assessing the patient, family/significant others level of understanding/acceptance of plan.
• Demonstrates the ability to process information on the plan of care and provide individualized, ongoing teaching to the patient and family/significant other throughout the continuum of care.
• Demonstrates the ability to process and interface appropriate information between the healthcare team and the patient, family/significant other to ensure quality patient outcomes.
• Develops individualized exercise treatment plans covering intensity, frequency and duration of exercise in conjunction with the Medical Director. Performs all treatment and procedures in accordance with established policies, procedures, and patient care standards.
• Plans and implements patient activities (i.e., functional exercise testing) to promote patients physical and mental well being.
• Communicates and follows-up with patients to assess progress, level of understanding of instructions, overall patient status. Maintains appropriate records.
• Accurately documents pertinent information on all required forms, to ensure optimal patient outcomes.
• Provides to patient the following services, when appropriate: administration, monitoring, pre and post assessment, teaching and evaluation of the patients response to the medication administered via any route
• Assists in developing and coordinating Cardiac Rehab patient and family classes. Performs any other duties as assigned.

Education:

• • Graduate of an accredited school of nursing

Experience:

• • 1-2 years of experience in Cardiac Rehab program or critical care area

Training/License:

• • Current NYS License

Other:

* ACLS required; BLS Certification
