St. Peters Health Partners is seeking a full-time Nurse Practitioner or Physician Assistant to join Palliative Care Partners, a service line across St. Peter's Health Partners supporting patients and their families who are dealing with serious illness, located in the Capital District, New York.
The selected Advanced Practice Provider (APP) will provide palliative care services in the community / home settings including home visits, tele-medicine, on-call rotation, and collaboration with the interdisciplinary team.
This role will work closely with an interdisciplinary team including a physician, Social Worker, and Spiritual Care support to provide comprehensive care to patients with advancing illness primarily in the community setting. The APP will work with consultant and practice staff to manage pain and symptoms, address goals of care and treatment plans and provide guidance and education to patients and families. We provide a supportive work environment and thorough orientation.
The schedule for this opportunity is Monday - Friday. Travel is required for this outpatient role.
Preference will be given to candidates who are board certified in Hospice and Palliative Care. Two years of clinical experience, preferably in palliative care and/or home health is preferred.
Candidates should possess excellent clinical skills, communication skills, and a strong commitment to providing excellent care that is team oriented. Board Certification, New York licensure and DEA are required.
