Nurse Practitioner / Physician Assistant - Primary Care Rockwood Clinic is seeking experienced Physician Assistants or Nurse Practitioners to join our team- based care model with our dedicated Family Medicine and Internal Medicine providers at our expanding primary care locations. Our team-based care model pairs one or two Physicians with one or two APPs based on patient panel size and FTE to formalize a Care Team. Instead of a singular PCP, patients can identify with an entire team of providers aligned with a formalized process designed to improve access, promote timely responses to results and MyChart messages, improve quality outcomes, and improve professional fulfillment and sustainability for our providers.Why Choose MultiCare?

Collaborate on consistent approach to patient care.
Prioritize in-basket support based on patient needs and care team preferences.
Maintain adherence to standard team-based schedule template.
Engage with Triage RNs to provide clinical decision support for disposition inquiries.
Regularly review panel size to preserve access for new and existing patients. Participating Physicians:
Serve as supervising physician for team-based PAs.
Work collaboratively with care-team to manage all aspects of patient care. Participating APPs:
Utilize dedicated population health time to manage the team in-basket.
Incorporate care gap closure into every patient encounter. Requirements:
1+ year of clinical Nurse Practitioner/ Physician Assistant experience
Washington State license by time of employment
Board certified/eligible at time of employment
DEA, NPI and prescriptive authority
