Responsible and accountable for the ongoing teaching, data collection regarding the condition and care, safety and monitoring of chronic hemodialysis patients as well as providing support and education to the patient's family. Functions as an intricate member of the multidisciplinary team.

Education:

• • High School Diploma/GED
  • Graduate from a Licensed Practical Nurse Program

Experience:

• • 1 yr. Hospital sub-acute experience and 1 yr. Hemodialysis experience preferred.

Training/License:

• • Current NYS Licensed Practical Nurse
  • BLS Certification through the American Heart Association
