The Registered Nurse - Patient Aligned Care Team (PACT) is a member of the Patient Aligned Care Team. The RN - PACT is responsible for collaborating with services internal and external to the VA to facilitate care transition in order to effectively meet the patients' needs. The RN - PACT provides patient and family health education with a focus on self-management, prevention, and wellness, based on the patient's goals. Major duties and responsibilities include, but are not limited to: Demonstrates leadership by serving as an advocate for patients, and a team leader with colleagues as she/he continues to enhance his/her own and the team's professional growth, development and practice. Uses the nursing process and evidence-based practice to collaborate with the Teamlet (Veteran, Provider, RN Care Manager, Clinical Associate and Clerical Associate) and expanded Team (Family/Caregiver, Social Worker, Pharmacist, Dietician, Internal and Community-Based Services involved in providing care to the patient) in developing the patient driven care plan. Leads and organizes education and delivery of care to assure continuity of care and peer accountability for practice, including access to care and discharge planning. Uses advanced clinical knowledge/judgment to promote staff involvement in planning, decision-making, and evaluating outcomes. Functions as an expert in clinical practice and/or areas related to the assigned roles and responsibilities. Systematically evaluates current practice, and formulating outcomes for groups of patients and/or organizational processes within an area of expertise. Guides, develops, and supports staff from a leadership perspective. Uses professional standards of care, scientific evidence, and evidence-based practice to evaluate programs and service activities. Works in partnership with an assigned panel of patients and teamlet members for effective disease and chronic care management. Makes caring for the Veteran his/her priority while in the work setting and demonstrates customer service principles in all aspects of work.
