The Salem VAMC is looking for an enthusiastic RN to join the Home Based Primary Care (HBPC) Team! This HBPC RN will be the Case Manager for Veterans within the Lynchburg, VA area and will be accountable for the provision of direct patient care and for delivering quality nursing care to the Veteran at their home. The RN must possess the knowledge and skills to effectively apply all aspects of the nursing process in the management of acute and chronic illnesses and support of the patient. Some duties and responsibilities for the position include but are not limited to: Provides a full range of nursing care to patients in home setting utilizing the nursing process. Completes initial assessments and reassessments of patient condition and response to treatment and other clinical findings; initiates follow-up as needed. Assesses patient's learning needs, develops and implements a plan to meet needs, and instructs patients and families in self-care and treatment measures; modifies approach as indicated. Promotes continuity of care through developing an individualized interdisciplinary plan of care in collaboration with the patient, family and HBPC team; involves community resources as appropriate. Serves as a resource for patient self-determination and acts as a patient advocate when patient self-determination is in question. Maintains professional boundaries with patients, families, employees, and vendors. Serves as a patient advocate, in protecting the health, safety and rights of patients. Promotes practices that enhance Veteran access to care/services, and promotes cost effective use of resources. Utilized established guidelines in accordance with functional statements, performance standards, position description and competencies to evaluate practice. Demonstrates effective communication skills and professional behavior that promotes cooperation and teamwork with internal and external customers. Consults with other health care providers to formulate a collaborative plan of care. Effectively communicates information to appropriate staff in a timely manner, resolving patient care issues. Demonstrates leadership in HBPC quality improvement activities. Identifies opportunities to improve patient care through monitoring, analyzing and evaluating care outcomes. Communicate and interact appropriately and courteously with all internal and external customers. Maintain confidentiality of electronic, written, and/or verbal patient/employee information. Demonstrate working practices that include adherence to Infection Control standards and the safe use and operation of equipment. Conforms with all medical center and service/service line safety policies, procedures and protocols to avoid injury and/or illness to self and others.
