The James E. Van Zandt VA Medical Center consists of a parent site, located in Altoona, PA and 5 community based outpatient clinics (CBOCs) located in DuBois, Huntingdon, Indiana, Johnstown & State College, PA. The medical center serves Veterans from a 14 county area in Central PA, providing exceptional health care in the areas of Acute Care, Behavioral Health, Diagnostic Services, Urgent Care, Long Term Care, Pharmacy, Primary Care, Social Services, Specialty Care & Women's Health. Associate Chief Nurse Acute & Long Term Care: Is accountable for day-to-day operation and management of the following Non-Institutionalized Long-Term Care (NILTC) programs: Medical Unit 4, Urgent Care Clinic (UCC), Nurse Officers of the Day (NODs) and Community Living Centers (CLC) 5 & 6. All programs are located at the main hospital in Altoona, Pennsylvania. The ACN provides executive leadership and direct supervision to the Nurse Managers over the Acute & Long Term Care Service. Assesses, plans, and evaluates the processes and daily operations of the NILTC programs. Coordinates inspections with clinical survey agencies. Follows up on sentinel events and other adverse events that occur in respective areas. Reviews EPRP, VERA reports, performance measures, SAIL, and other data sources for trends and early intervention; formulates follow-up plans with service and other services as necessary. A few specific measures watched closely for performance are length of stay days, admissions, discharges, short term stays, restraint days, fall & fall prevention, pressure ulcers & pressure ulcer prevention, hospital acquired pneumonia preventions, just to name a few. In collaboration with the Associate Chief of Medicine Service, designs and implements interdisciplinary programs within the service and the Altoona VA Medical Center. The Associate Chief Nurse serves as a member of Patient Care Services Executive Council, Nurse Practice Council, and VISN committees as assigned to ensure nursing practice and operations are consistent with the Joint Commission Nurse Care Center standards. The ACN will be responsible for interpreting VHA, VISN, and Altoona VA Medical Center performance standards, regulations, policies, and VHA Directives to employees. The incumbent will coordinate with the order to ensure compliance with standards and other regulatory bodies for all program areas. The incumbent ensures nursing staff compliance with all internal and external regulatory body standards, as well as the VHA performance measures.
