MEDICINE DIVISION Registered Nurse - Referral Coordination Summary The Referral Coordination Initiative (RCI) Registered Nurse (RN) Care Coordinator (CC) is responsible for the processing of pending Clinical Consult referrals in accordance with VHA Scheduling Directive for the Medical Subspecialty services. Under the supervision of the Nurse Manager and in collaboration with the Deputy Chief of Medicine, the RCI RN CC serves as a liaison to providers, manages the seamless expediting of care across a continuum, and provides options for Veterans seeking care through the VA. The goal of the initiative is to allow the Veteran to make the most informed decision for the Right Care, at the Right Time, in the Right Location (when eligible). Job Duties /Expectations: The Registered Nurse duties will include but are not limited to: Coordinates visits according to the needs and abilities of the patient. Determines Veterans enrollment status Performs assessment of referral based on clinical triage Contacts Veteran and provides options based on wait time and community care eligibility If Veteran chooses community care RN will obtain preferences and forward to that department Works closely with specialty providers at the medical center to overbook if there is not availability Educates medical center staff on Referral Coordination Initiative Demonstrates consistent clinical competence in providing nursing care Demonstrates evidence of critical thinking skills in identifying, analyzing, and resolving patient care concerns Provides leadership in establishing goals, evaluating the plan of care, and in discharge planning Initiates referrals to other disciplines and/or services as appropriate Assesses the patient's learning needs, learning preference, and readiness to learn; modifies teaching approach based on knowledge of human behavior, motivations, and reactions to situations Coordinate and support the needs of the Veteran requiring multidisciplinary teams in facilitating the disposition of a specialty consult Remove non-value-added steps to optimize the use of clinical staff for the benefit of the Veteran Maximize the interaction with the Veteran to identify needs and gather preferences to ensure that appropriate modality of care is offered Identify possible barrier to care and with the Veteran, family, and clinical care team to provide a team-based plan of care Enhance the quality of clinical outcomes and Veteran satisfaction while offering the Veteran the opportunity to make an informed decision about their care Work Schedule: Full time Days Preferred Qualifications: Virtual: This is not a virtual position.
