The Cincinnati OH VA Medical Center is seeking well-qualified candidates to work as a Registered Nurse Care Manager in the COVID/Influenza Vaccine Clinic. The RN in this position will be part of the Patient Aligned Care Team (PACT) and will be over ceiling to work in the COVID/Influenza vaccine clinic and swabbing for unvaccinated employees. Duties and Responsibilities include, but are not limited to: Utilizing the nursing process and evidenced-based practice, the RN Care Manager Collaborates with and leads his/her assigned PACT Teamlet in the development and implementation of the patient driven, holistic care plan for the veteran. Develops the plan for health education and assures the patient and his/her family receives the information needed focusing on self-management, prevention, and wellness, based on the patient's goals. Is responsible for collaborating with services internal and external to the VA to facilitate care transition in order to effectively meet the patients' needs. Works with members of the expanded health care team and consultants as the patient's goals dictate. Provides education, management and monitoring of chronic and acute disease processes and chronic pain. Utilizes Telehealth modalities to deliver patient care. Makes assignments and provides direction to the other members of the nursing staff. Serves as a preceptor and mentor to colleagues and students, enhancing the professional growth, personal development and practice of others. Demonstrates leadership by active involvement in area-based councils, committees, and task forces. The RN Care Manager is directly supervised by the Primary Care Chief Nurse or Nurse Manager assigned to the area in which he/she works.
