Fairview Health Services is looking Registered Nurse to work out of the Maple Grove Ambulatory Care Center in Maple Grove, Minnesota. This position is approved for 80 hours every two weeks and is benefit eligable.
The RN delivers care that is sensitive to the unique needs of the care provided by the Acute Diagnostic Services (ADS). Delivery of care may include but is not limited to peripheral IV initiation and management, order management, medication administration, simple wound care, performing and assisting with procedures, monitoring response to interventions, prompt recognition and intervention of hypersensitivity reactions, preparing and delivering patient education, monitoring and taking vital signs, documentation phone triage, coordination of care including rooming and discharging, and monitoring standards of care.
Assesses and cares for patient with acute conditions and identifies expected nursing outcomes. Involves the patient, family and other health care team members in using a systematic process to gather and analyze data to identify needs and potential risks.
Develops plan of care and documents in collaboration with provider(s), other health care team members, patient and family.
Delivers care as delegated by provider to individual patients and documents in medical record.
Performs technical procedures and medication administration including: Clean and Sterile Procedure set-up, IV insertion, removal and management, IV Push, IV hydration, IV antibiotics, injections, basic wound care, wound cultures and wound irrigation, nebulizer treatments, splint application, point of care (POC) testing, Foley and straight catheter insertion, patient teaching of Lovenox and other self-injectables as defined by the ADS setting.
Evaluates care and the patients response to interventions and expected outcomes.
Provides referrals to MTM, Care Coordination, BHC, financial counselor, advanced care planning resources, etc.
Receives notification and relevant information from patients PCP care team for either a Day of Diagnosis or First Order Acute and transitions patients care back to PCP and care team.
Demonstrates all critical competencies per ADS skills checklist and new required learnings.
Bachelors degree in nursing
Minimum of 2 years of acute care nursing
Experience with an electronic medical record
Valid Minnesota Registered Nurse license
Maintain American Heart Association Basic Life Support (BLS) Certification
Bachelors degree in nursing
Preferred: 2-3 years of acute care nursing and/or infusion or ambulatory urgent care nursing experience
