The Community Care Clinic (CCC) cares for Veterans receiving care in the community when Veteran meets community care eligibility criteria. The CCC is the primary avenue for all administrative and clinical functions associated with the Community Care referrals. The effort focuses on the Veteran journey and experience by striving to provide choice and accessibility to community providers and high quality, timely, and standardized care. Duties Include but not limited to the following: The Community Care Nurse Manager is accountable for the operation of the Community Care Clinic and the care needed to support the patient utilizing community to obtain inpatient, outpatient and home care. Responsible for the planning, integrating, implementing, modifying and evaluating the effectiveness of the Veteran's care. Will serve to support delivery of high quality, timely care to the Veteran receiving services in the community. Functional role includes following the standardized care coordination model, incorporating case management principles if applicable, knowledge of processes related to referral and authorization and secure medical documentation exchange. The ability to identify Veterans requiring disease management and the process to obtain these resources internal or external to the VA if applicable. The RN will focus on providing care that is unique to the Veterans needs and continues to monitor, track and intervene if necessary in the episode of care across the continuum. The Registered Nurse will demonstrate effective communication skills to collaborate and co-manage the Veterans care with the multidisciplinary team in a cost-effective manner. Designated first-line supervisory authority. Is responsible, and accountable for the operations of their unit. Has continuing responsibility for time scheduling of the unit, coaches staff, addresses employee conduct and performance issues, completes recurring reports and performance evaluations in a timely manner. Participates on service and medical center committees and task forces to improve nursing practice and patient care. Identifies educational needs of staff and ensures that these needs are addressed.
