Summary

Provides a significant level of patient education related to their illness and planned treatment in inpatient setting.
Responsible for guiding patients with cancer DRGs through complex care continuum and ensuring appropriate handoff to outpatient setting.
Oversees outpatient Nurse Navigation to facilitate smooth transitions for patients and standardization of resources.
Assures patients receive care that consistently follows evidence-based standards and is in accordance with regulatory standards as defined by regulatory agencies.
Interacts with community resources and develops relationships with service providers to enhance patient experience and eliminate barriers.
Participates in development and implementation of strategic business plans for Survivorship Program to achieve financial and operational results as well as a sustainability model.

Education

Requires Master of Science Degree in Nursing from accredited college or university.

Experience

5 years clinical in oncology nursing and 3 years in Nurse Navigation. Prefer more than 5 years clinical in oncology nursing and more than 3 years of oncology nurse navigation including cancer survivorship care.

Certifications

Current state RN License, BLS from American Heart Association or Military Training Network. Prefer Certified Breast Care Nurse (CBCN), Certified Breast Patient Navigator (CBPN-IC), and Oncology Certified Nurse (OCN).
