EO/AA Huntsman at Home is a new and innovative initiative to patients in their homes. This is at the leading edge of palliative care and is strengthened by being integrated with the hospital. We are looking for experienced APRNs interested in providing care, equivalent to an inpatient unit, in the patients’ homes. The team will provide multidisciplinary oncology care, palliative care, and hospice care to HCI patient’s in their homes. This team will function 7 days a week to help reduce the length of inpatient stays while continuing to provide high quality and cost effective care. Immediate potential for leadership roles is also expected. Working as part of the multi-disciplinary team (MDT), the APRN will be involved in the development, implementation and evaluation of a seamless specialist service, ensuring that patients receive the highest standard of care in their own home. The APRN will demonstrate a high level of expertise within the palliative care field in assessing patients, developing and implementing a care plan. The team will be responsible for advance care planning and goals of care conversations with patients and families. They will be responsible for contributing to the ongoing development of clinical practice, research and standards of care within the service, as well as policies, protocols, guidelines and service developments. They will ensure the safety and well-being of patients and work in a manner that promotes dignity and human rights through patient centered care principles.
