The Licensed Vocational Nurse (LVN) is a non-exempt position responsible for performing a variety of duties depending on whether it is a small clinic, large medical practice, multi-clinic, or a specialty office. The LVN functions under the supervision of the Nurse Manager/Lead Nurse. The LVN assures provision of care to an assigned patient population, utilizing the principles of Performance Improvement in collaboration with other health team members. The LVN supports an organizational culture for Service Excellence and practices the Service Excellence Standards to all customer groups.

Northwest Texas Physician Network is recruiting one ACO LVN for the the Wyatt Clinic. The role of the ACO LVN is to maximize the components of care coordination to improve patient's healthcare experience and outcomes, by utilizing care coordination tools, criteria and protocols to provide patients with chronic and acute conditions that support, educate and assist in the prevention and/or maintenance of their disease and/or health and wellness state; increase patients compliance with treatment plans; engage community resources to support the patient's optimal functioning; and improve collaborative coordination of care to affect waste and inefficiency. implements and notes response to an age-appropriate plan of care utilizing assigned components of the Nursing Process, Clinical Skills, and Patient Education. The LVN delegates to unlicensed personnel in accordance with Nurse Practice Act. Also, the LVN provides an environment conducive to safety for patients, visitors, and employees. The LVN will report risks to patient safety and implement the appropriate Precautions. The LVN will comply with appropriate and approved standards.
