Christus Integrated Care is becoming the standard care delivery model, within the med/surg and telemetry units, across our acute care facilities.
This model is built upon a team of three caregivers, a RN, LVN and CNA, working in collaboration, which is designed to maximize the amount of actual care hours available to our patients.
Feedback from the staff:
"I like the teamwork. If my LVN gets a lot of PRNs, she can ask me to help with those. We all work together."
"There is less "missed" care such as ambulation. I never had time to ambulate patients before."
"The patients were very pleased to learn they would have 2 nurses taking care of them!"
"All day long, I was ahead of schedule with things that needed to be done."
"We all got to lunch on time."
RNs are expected to practice at the top of their licenses, coordinating the care of the patients and providing direct hands on care while directing the activities of their two other team members.
LVNs/LPNs function under the direction of the RN and are not assigned directly to patients.
The LVNs/LPNs continue to be responsible for their practice. They must maintain their competencies, practice within those competencies, and follow best practice guidelines as well as hospital policies and procedures.
