Summary of Job Functions:
The Patient Navigator is an integral part of the care coordination team and is responsible for collaborating with patients, families, caregivers, physicians and other members of the multidisciplinary team to coordinate and help manage ACO patients' care. The intent of patient navigation activities is to ensure that patients are able to access all appropriate services in a timely and cost effective manner. The Patient Navigator will work to overcome obstacles and guide members through the healthcare system. The patient navigator also identifies patients in need of navigation who have not yet been assessed.
This role requires someone who has empathy, compassion and an understanding of the cultural and socioeconomic backgrounds in the community and is knowledgeable about community resources, including financial, educational, social, and emotional support services available to patients.
Experience in serving the populations that will be referred for patient navigation services.Experience in developing strategies to overcome barriers that prevent clients from accessing services and completing services.Knowledge of both community and healthcare system resources. Excellent interpersonal skills to instill confidence and to advocate for patients and their family. Exceptional organizational skills and ability to keep well written documentation. Ability to build professional relationships among members of the interdisciplinary team. College degree preferred but not required
