Nestled in the farmlands of Middlebury,Vermont, Helen Porter has accommodations for 105 people, is licensed by the state of Vermont and certified by Medicare and Medicaid. Helen Porter is a skilled and intermediate care residence which specializes in providing a home-like environment including:
Expansion and renovation of post-acute unit, rehab gym and new end-of-life care suite are planned for this spring
Affiliation with academic medical center network provides opportunities to collaborate with peers as organization seeks to become an integrated healthcare system of choice
Community & mission focus drives Helen Porter to provide high-quality care for the most vulnerable members of the community, especially via:
Onsite MD/NP provider team
End-of-life care partnership with ARCH – Addison Respite Care Home
24/7 accountability for nursing functions of the organization in accordance with all applicable state and federal regulations
Responsible for assuring an appropriate quality monitoring, and evaluation within the Nursing Department in conjunction with the overall facility Quality Assurance Performance Improvement Plan
Develop and maintains department objectives, standards of nursing practice, policy and procedure manuals, job descriptions for all nursing team members.
Act as role model throughout the nursing department
Participate in recruitment and retention of nursing team members in coordination with Human Resources, and Fiscal Services
Prepare and adhere to nursing services annual budget
Maintains adequate nursing records for clinical and administrative use
Represent the facility to residents, physicians, family members, and referral agencies
Foster a collaborative approach in the delivery of care and services throughout the organization
Demonstrated ability to:
Build trust and develop relationships with diverse groups
Solve problems with innovative solutions in a collaborative manner
Act as resource of clinical expertise
Lead change with a thoughtful approach
Coach and educate team members to provide the best patient and resident experience
VT RN licensure
BSN Required, MSN Preferred
At least 2-3 years of nursing leadership experience, preferably in a long-term care or post-acute setting
