Under the direction of the Clinical Manager or RN Supervisor, this position is responsible for the planning and the managing of the nursing care of an assigned caseload of patients in Hospice Care.
As a member of an assigned care team, the RN coordinates resources and performs a wide range of skilled nursing activities for an assigned caseload of adult patients. Responsible for ongoing assessment of patient/caregiver needs, symptom management, teaching of appropriate care related to disease process, and participating as a member of the interdisciplinary group (IDG) in providing end-of-life care/support for patients and caregivers alike.
All employees assigned this job title must be willing and able to flex their work location within and between any Community Hospice location.
Any combination of education and experience that would provide required skills and knowledge for successful performance would be qualifying. Typical qualifications would be equivalent to:
Possession of a degree from an accredited school of nursing program, 2 (two) year Associate Degree, or a Bachelor’s Degree in Nursing.
One (1) year Registered Nurse experience with one (1) year hospital medical-surgical/oncology experience preferred or
Five (5) years Licensed Practical Nurse experience within the past five (5) years, with one (1) year hospital medical–surgical/oncology experience preferred.
Knowledge of appropriate educational principles and methods as applied to hospice nursing.
Effective communication and teamwork.
Demonstration of confidence and ability to manage inpatient facility functions and patient care needs on his/her shift.
